SG/PRC is guided through a person-centered approach when assessing the individual needs of your loved one. We connect you and your loved ones to an array of services and supports uniquely designed to meet their specific needs.

SG/PRC is dedicated to ensuring that you and your loved ones receive services and supports that will help you achieve your dreams through independent, productive lives guided from your choices and preferences.
We also believe in celebrating your victories and encouraging you throughout your journey. SG/PRC strives to empower the person served and their families to decide where they want to live, whom they want to spend their time with, what relationships they wish to foster and to decide and choose services that best support them. The journey with SG/PRC begins through the eligibility process. Once eligibility is established you and your loved one will be connected to a Service Coordinator. The Service Coordinator will support you in securing regional center funded services and community resources.
Under the leadership of Daniela Santana, Director of Client Services, she and her team of Associate Directors and Managers develop policy, procedures, and best practices. As a team, they ensure efficient administration of service coordination resources.
