A SIR is a formal report that is filed with the regional center whenever a person served by the regional center is involved in an unusual event, such as a crime or injury.
There are detailed and specific rules about Special Incident Reports that service providers and regional centers must follow whenever a special incident occurs.
Special Incidents Reports (SIRS) must be reported to a regional center within twenty-four (24) hours of the incident occurrence. SIRS can be reported directly to the service coordinator by calling them on the telephone. However, if the service coordinator is not available to answer your phone call do not leave a voicemail. Due to the sensitivity of SIR’s, there are three (3) alternatives for you to report the incidents.
